Instructions:

1. Right click on one of the green links above
2. Select "Save As"
3. Save to your local computer
4. Open and edit with Microsoft Excel (c)
5. Save file
6. Attach file to email and send to Pastor Sandra

Don't have Microsoft Excel?  Open Office is an open source software that can be downloaded and installed on your computer which will allow you to view, edit, and save documents created in a Microsoft Office format. Please remember to save your forms with an extension of .xls if you use Open Office.  You may download the software for free at their website http://www.openoffice.org/
